We sell this item in booklet form so we do not have an actual separate instruction page to send out? It all comes together in one booklet. You could contact the designer/manufacturer, Kooler Design Studio, and I am sure they will send you just the instruction page that you are missing. Here is there website:

Did you purchase just the chart or did you buy a kit? This pattern was sold both ways. I have a Janlynn kit and I know that Bucilla has made a kit also. Depending on the company you may find the color symbols and or floss codes may be different.
