I posted earlier on your post but in case you don't see it - I have the pattern and cover picture of the pattern you are looking for. Unfortunately I don't have the threads or fabric but if you are an experienced stitcher there is a list on the back of the pattern that gives the color name that you could match with DMC colors. I believe it is a Dimensions pattern so possibly they could match you up with the numbers and threads.
